Credit Monitoring Services

A Class Action Settlement was approved on August 16, 2018 by the Court for the Northern District of California in litigation against Anthem, Inc. and other Defendants relating to the Data Breach that Anthem announced in February 2015. Pursuant to the Settlement, Experian is providing Credit Monitoring Services to Settlement Class Members who filed a claim by July 19, 2018, and Fraud Resolution Services to all Settlement Class Members who request such services, even if they did not file a claim for Credit Monitoring Services.

CREDIT MONITORING SERVICES

If you have an activation code, click here to enroll in free Credit Monitoring Services, or call toll free 1-866-579-2216.

If you filed a valid claim for Credit Monitoring Services before the July 19, 2018 deadline, then you should receive an Activation Code from the Anthem Data Breach Settlement Administrator by email or mail, with instructions on how to activate the Credit Monitoring Services.  You will need your Activation Code to enroll.

If you submitted your claim for Credit Monitoring Services under the Settlement before July 19, 2018, but you have not received your Activation Code and enrollment instructions by December 14, 2018, please call 1-877-703-0745 for assistance.

FRAUD RESOLUTION SERVICES

If you have suffered an identity theft incident, and would like a fraud resolution specialist to help you resolve it, please call Experian toll free at 1-866-579-2216. To obtain fraud resolution services, you will need to provide Experian with an Engagement Number.  You can obtain your Engagement Number from the Settlement Administrator at www.DataBreach-Settlement.com or 1-855-636-6136.

All Anthem Settlement Class Members, even those who did not file a claim form, are entitled to Fraud Resolution Services. You are a Settlement Class Member if you were sent a notice about the Data Breach, and/or your personal information was taken during the Data Breach.
